Student Opinion Survey Form
Near the end of every semester, faculty at UGF asks students to evaluate both the course and the instructor fairly and honestly. The surveys are confidential. A Program Assistant will write a report about the survey and any comments made about the class. The instructor will see only the report and not the surveys themselves.

STUDENTS: In the interest of improving the quality of instruction at the University of Great Falls, we value your honest opinions and perceptions about your experience this semester. Please consider carefully your responses to this evaluation. Your responses are anonymous and confidential. Your instructor will not be shown results until after the semester has ended and grades have been recorded.

Please use the TAB key or mouse clicks to advance from question to question. Using the Enter key may accidentally submit the form prematurely.
